Thanks for the images. I completely understand that the situation seems weird.
You did not mention, but I suspect it is a Samsung hard disk.
For this particular Samsung hard disk model, recently we discovered some issues which resulted in lower health, as you can see. Version 4.50 of Hard Disk Sentinel checked and reported lower health when a special combination of attribute changes (which may lead to problems) detected.
After investigating more drives from this model, it is confirmed that the status is fine and the slightly increased attribute values do not really indicate problems. So an updated version is available which shows the 100% perfect status (again) unless there is any real problem with the hard disk drive.

Please just close the current version by File -> Exit and install this to the same folder, without uninstallation. Then the updated version will
automatically use all settings, statistics, registration details - and it should display complete status immediately - and should show 100% health
without any kind of changes/adjustments.
I'd suggest to please use Report menu -> Send test report to developer option with it in all cases: if things are better, it would help to verify the situation but if not, it helps further development, helps to check the actual hard disk model ID, firmware version and S.M.A.R.T. status together.
Also, just to be sure, you may consider perform some testing with Hard Disk Sentinel, as described at

I'd suggest to use
1) Disk -> Short self test
2) Disk -> Extended self test
3) Disk -> Surface test -> Read test
as these are compltely safe for the stored data and these would confirm if the status is really error-free or reveal any kind of problems accessing the hard disk sectors or by the hard disk in general.
