I have a hdd with problems as you can see in the attached picture.
I am during a Surface test process and at the moment I see that everything looks ok.
my question is to keep the hdd in operation or to get rid of it?
I wouldn't want to have any surprises.

The surface test window is not complete, so hard to say anything for sure.
Ideally if the surface test shows no errors (so all possible bad sectors found, reallocated) then the drive can be still used with constant monitoring to be informed about any (even minor) new issue.
However, considering the low health % there is good chance that new and new problems, new bad sectors will appear (similarly as you just had a health decrease from 13 to 12 now).
The drive may be ideal for secondary storage, but personally I'd surely not use for critical data.
